We are in fact required by law to keep mum about such matters; the only time is when it is clear they are putting themselves or someone else in harm's way. See if your practice has a lunch break - go right after, so the doctor shouldn't be held up by anyone. Or make a first-morning appt. schedule is the one that has the most amount of evidence and data to back it up. The "alternate" or "spaced-out" schedule, on the other hand, does not. So I would be doing a child a disservice if I use a schedule that I don't know the safety profile to. Logically speaking, it should be ok to give shots separately if it's ok to give them together. However, I don't have data to back up that claim. That being said, I will space them out if a parent feels very strongly about it, even after discussion (and I inform them of this lack of data on the spaced-out schedule), because I would rather that baby or child vaccinated against these preventable diseases than not.

My own kid needed 3 grown-ups to hold him down when he got his 4-yo shots. Then he whined and cried for 2 hours after. I usually get them done and over with before I even start the visit, so they're not so scared. And we also have tons of stickers, toys, and (yes) lollipops in the office to reward them after they get their shots. For the parents, I wouldn't tell them until that day, unless you have the kind of child who would rather know and would get mad if you didn't tell him ahead of time.
